Health Forms for Scouting Events
General Health Form A & B – This is used for general Scouting events up to 72 hours.
Camp Health Form A/B/C – This is used for camping trips or events lasting longer than 72 hours.
PADI Form & Sea Base Health Form A/B/C – Sea Base has its own health form. If a Scout or Scouter intends to scuba dive, the PADI form is also required.
Northern Tier Health Form A/B/C – Northern Tier High Adventure Base has its own health form.
Philmont Scout Ranch Health Form A/B/C – Philmont Scout Ranch has its own health form.
Philmont Training Center Health Form A/B/C – Mountain trek, Trailblazers, Mustangs, NAYLE, NAYLE staff, PLC, and PLC staff use this health form.
Philmont Training Center Health Form A & B Limited Backcountry – PTC conferences and all other family program groups can use the limited backcountry download.
Summit Health Form A/B/C – Summit Bechtel Reserve has its own health form.

Guide to Safe Scouting – This is the online version of the GSS, which is the most up to date. You can also download a PDF version.
Incident Information Report – Use this tool to report all injuries, illnesses, and incidents during Scouting activities or on council-owned properties that require the intervention of a medical provider beyond basic Scout-rendered first aid.
Youth Protection / Membership Infraction report – Use this tool to report all allegations of abuse, violations of BSA guidelines or policies, inappropriate behavior by a Scout/Scout leader/parent/other.
Near Miss Incident Information Report – Use this tool to report near misses in programs such as COPE and climbing activities. A near miss is an incident that did not result in injury, illness, or damage by definition but that had the potential to have done so.
